Support Coordination for Families with Substance Use Disorders (Fauquier LINK)

Through an interagency collaboration with the Fauquier County Department of Social Services, this program provides community-based services for children and their families affected by a parent(s)’ substance use. With a goal of reducing the number or length of foster care placements associated with parental substance use, our support coordinator provides intensive case management, home visitations, and support services to coordinate and enhance the effective utilization of community resources. Services are recovery-oriented, strength-focused, and person-centered.

Individuals must be referred through Fauquier County Family Assessment and Family Planning (FAPT).

Services may include coordination with the following community resources:

  • Substance abuse prevention and treatment
  • Family planning
  • Prenatal care
  • Well-baby care
  • General health care
  • Developmental screening
  • Assessment and intervention
  • Child protective services
  • Foster care
  • Parenting education
  • Public education
