Intellectual Disability Case Management

Case Managers work with adults who have an intellectual disability and their families to:

  • evaluate what the individual needs in order to remain successful in the community;
  • link the individual to programs and supports that are needed or requested;
  • monitor the supports given to the individual to ensure they are meeting his/her needs; and
  • advocate for other services that will help the individual in the community.

Case Managers help coordinate services to meet medical, psychiatric, social, nutritional, educational, vocational, employment, housing, transportation, recreational, legal, and advocacy needs. Intellectual Disability Case Management Services are the entry point to all RRCSB-AAA residential and day support programs.

Eligibility for Intellectual Disability Case Management Services is based on diagnosis and an identified need for Case Management Services (for individuals age 6 and older). There must be documentation that the individual has a diagnosis of intellectually disabled as defined by the Code of Virginia: “A substantial sub-average general intellectual functioning which originates during the developmental period (before 18 years) and is associated with an impairment in adaptive behavior.”

A psychological evaluation must be completed by a licensed professional, contain a diagnosis of intellectually disabled, and reflect the individual’s current status. In order to confirm a diagnosis of intellectually disabled, the evaluation addresses intellectual functioning, adaptive behavior, and age of onset. This evaluation must be completed prior to the initiation of the service.
